New Patient Forms
Once you book your appointment, we'll text and email your new patient forms directly to you — no downloads or printing needed.
Fee-for-Service Practice
We are a fee-for-service practice that works with most PPO insurance plans. We bill your insurance directly and collect reimbursement on your behalf — in most cases you simply pay your remaining balance at the time of your visit.
Financing Options
We offer CareCredit and flexible payment plans to make your investment in your smile more manageable.

Insurance Questions
How Our Insurance Process Works
We make insurance simple. Here's what to expect:
- We bill your PPO insurance directly on your behalf
- We collect reimbursement from your insurer — no waiting, no claim forms for you to file
- You pay only your remaining balance at the time of your visit
- Delta Dental patients: please note that Delta Dental reimburses patients directly rather than our practice — our team will walk you through what to expect at your appointment
